Survey for People who receive or are contracted with Community Living BC

Hi if you are a family, caregiver, supported person who is funded through Community Living BC (CLBC) can you please fill out the survey below it would be greatly appreciated. Home Share Caregivers of BC are meeting with CLBC at the end of the month and would like to share this information with them. This survey is confidential. If you have that is great! If you could share this survey with other caregivers and families that would be great as we want as much input as possible. We are also asking if you would like to share your experience with us about your dealings with CLBC or your agency that we could include in the report/presentation?. We will not use your name unless you would like us to. At this time CLBC doesn't seem to understand the problem around disputes and feel that it is rare for caregivers/families and agencies to feel they have been treated poorly.
